ISS incident July 2021

I didn't hear about this incident. Apparently it was a lot worse that was being reported? Anyone know much about it?

The problem is that Roscosmos is pretty much a shadow of its former self; workmanship and quality control are suffering, as well as morale.  That's leading to things like booster failures due to shoddy construction, errant holes drilled in spacecraft, and now apparently this incident; per the second Ars article, Rogozin claimed someone on the ground "could have made a mistake." 

Frankly, I do not believe Russia has the ability to support a manned space program anymore - like Boeing it's coasting on reputation and past glory, but it has lost the ability to do even the basic bread-and-butter stuff, much less adding new modules to the ISS decades after they were built.
